In the blank space for "Host Institution", please write down the full title of the host institution. No abbreviations are permitted. The full title of the host institution should be the same as the words on the school official seal.

In the blank space for "Name", please write down your Chinese name. In the blank space for "Family Name" and "Given Names", please write down your English name. If you don't have a Chinese name, write down your English name in the "Name" section. All the applicants must write down their English names.

Please fill in the blank space for "Nationality" in Chinese.

Please fill in the blank space for "Passport No." according to your passport.

In the blank of "Sex", please choose "male" or "female".

Please fill in the blank of "Marital Status" according to the date format of "Year/Month/date" in Arabic numbers.

Please fill in the space for "Place of Birth" in Chinese according to the format of "Nationality, (province)/city".

In the space for "Home Add. & Tel.", please write down your home address and telephone number in your home country. The home address can be written in English format. Please add the "country code" and "area code" in front of your telephone numbers.

In the space for "Highest Academic Degree Obtained", please write down the latest academic degree. If you haven't completed your study, please mark down your finished grade (For example: "sophomore").

In the space for "occupation", please write down your current occupation (For example: "student").

In the blank space for "Employer or Institution Affiliated", please write down the full name of your school or company you worked before the application.

In the blank for "Field of Study in China", please write down your objective major or courses in China. For example: Language/kindergarten/primary school/middle school/high school/technical school or the major name.

In the blank space for "Duration", please write down your study time from application to graduation.

In the blank space for "Student Status", please choose one of the following options: "new student", "transfer to another school", "enter a higher school", "validity extension", "guardian change", "passport renewal", or "others".

In the blank of "Deadline for Registration", please write down the deadline for school registration, and finished the school registration before the deadline. If not, applicants will result in immediate cancellation of school admission.

In the blank of "Reference & Tel.", please write down your referral person or unit's name and telephone number.

In the blank space for "Guarantor in China & Tel.", please write down your guarantor's Chinese name and telephone number.

In the blank space for "Source of Funding", please mark "√" behind the option you chose, and write down your guarantor person or unit's name in the blank space for "Financial support will be provided by". For example: choose "scholarship" in the blank of "Source of Funding" and write down the scholarship institution's name in the blank of "Financial support will be provided by".

In the blank space for "Signature of Applicant" please only sign on the first page. Since the form is made of carbonless paper, please pay attention to avoid your signature being sealed on later pages.
